Clients that are more than 15 minutes late for their scheduled appointment time cannot be guaranteed service and may be asked to reschedule. Please call 24 hours prior to your scheduled appointment for cancellations or rescheduling.
Client referral forms and registration forms may be faxed prior to the appointment or brought in with you to your appointment. We ask you to arrive 15 minutes prior to your scheduled appointment time. Be sure to bring any medical records, radiographs, and current medications.
For your convenience, we have provided the following forms in both MSWord and PDF format. The Client Registration Form is to be filled out by you, and the Referral Form is to be filled out by the referring veterinarian but is not required.
